Study of Medications Associated with AVN Onset

Goal
This study is compiling a list of any medications that patients were taking at the time of estimated Avascular Necrosis/Osteonecrosis onset. Once this information is compiled, it will be used in conjunction with results of other studies in a data mining effort that will hopefully yield patterns and connections with AVN.

Privacy
As with all studies on this site, your personally identifiable information is never divulged or distributed to third parties. Your optional participation will render completely anonymous data.
 
Instructions
Please answer the questions below as best you can. Though, be sure that your answers are limited to medications you were prescribed and actively taking immediately prior to and at the time of estimated ANV onset only. Do not list any medications prescribed to treat post-diagnosis symptoms.  If you require more than 5 medications, please submit the form as many times as needed to list all medications.
